Dr. Rey Varela is a recipient of an EHDN fellowship and the first physician to receive training in HD centers in Barcelona as part of the LA PEP program. Dr. Varela is a psychiatrist from Maracaibo, and a staff member of Factor-H who has been caring for the patients of the community of San Luis. This is a brief “diary” of his stay in Europe.

Part I – Parte I
My first week in Barcelona was one of adaptation, between getting used to the time change (6 hours apart), and the feeling of security. It is strange, but I did not longer remember feeling safe on the street, taking out my mobile without the fear of being robbed or taking the subway without fearing any danger. That feeling of security was so shocking, that, for a moment, tears came to my eyes in Barcelona. I was surprised by the entire public transport system, the clean and orderly streets, the friendly people with a conscience of multicultural citizenship, regionalist, but open to receive to people of so many nationalities.
Mi primera semana en Barcelona fue de adaptación, entre acostumbrarme al cambio de horario (6 horas de diferencia), y a la sensación de seguridad. Es extraño, pero no recordaba sentirme seguro en la calle, sacar mi móvil sin temor a que me roben o tomar el metro sin miedo a algún peligro. Esa sensación de seguridad fue tan impactante que por un momento se me salieron las lágrimas en Barcelona, me sorprendió todo el sistema de transporte público, las calles limpias y ordenadas, la gente amable y con conciencia de ciudadanía multicultural, regionalista, pero abierta a recibir a personas de tantas nacionalidades.
It all starts in 2019
The experience in Barcelona began, some time ago, when the Factor-H foundation encouraged me to apply for an EHDN fellowship (EHDN is the European HD network). In 2019 I received an email telling me that I was accepted. Then, before I could travel, the terrible pandemic begins and everything was postponed until the situation improved and the governments of the world could vaccinate and control the covid-19 pandemic.
In August 2021, with things getting better in Spain with the COVID issue, the EHDN committee already wrote to me asking if I was ready. I got in touch with those who will be responsible for my reception and guidance through the Fellowship in Barcelona: psychiatrist Dr. Jesús Ruiz Idiago, and with neuropsychologist Dr. Saul Martínez Horta.
Todo empieza en 2019
La experiencia en Barcelona se inicia, tiempo atrás, cuando la fundación Factor-H me alienta a postularme para la beca de la EHDN (EHDN es la Red Europea de la EH). En 2019 recibo el email que me comunica que fui aceptado, pero, antes de poder viajar se inicia la terrible pandemia y todo se pospone hasta que la situación mejorare y los gobiernos del mundo puedan vacunar y controlar la pandemia del covid-19.
En agosto del 2021, con las cosas mejor en España con el tema COVID, me escriben ya de la EHD para preguntar si estoy preparado y me pongo en contacto con quienes será el responsable de mi recibimiento y conducción por el Fellowship en Barcelona, el Dr. Jesús Ruiz, y con el Psiquiatra Saul Martínez.
A good flight
It is not easy to prepare a trip like this from Venezuela, a country with multiple socioeconomic problems. I was able to leave the city of Maracaibo, thanks to the management and alliances of Factor H and Hábitat Luz. I was very lucky, because most of the flights to Europe leave through Caracas with a stopover in Turkey but, it was possible, finally, to get a last minute flight from Maracaibo with a stopover in Santo Domingo, Madrid and, at last, Barcelona.
I left on September 27, with a return ticket to Venezuela on November 18. It will be a 52-day experience to remember.
Un buen vuelo
No es fácil preparar un viaje así desde Venezuela, un país con múltiples problemas socioeconómicos. Yo pude salir de la ciudad de Maracaibo gracias a la gestión y alianzas de factor H y Hábitat luz, fue toda una suerte, porque la mayoría de los vuelos hacia Europa salen por Caracas con escala en Turquía. Finalmente se logró conseguir un vuelo de último momento desde Maracaibo con escala en Santo Domingo, Madrid y, finalmente, Barcelona.
Salí el 27 de septiembre con retorno a Venezuela el 18 de noviembre (52 días).
Meeting with the clinical team
After that first week of adaptation, my first medical meeting was with Dr. Jesús Ruiz Idiago, who introduced me to the entire team of the Madre de Deu de la Merced Hospital, a multipurpose Health Center, where there is a whole clinical team working together: psychiatry, neurology, neuropsychology, social work, nursing, assistants, as well as personnel for specific tasks, such as cooking and cleaning. This team makes it possible, with the human and professional care provided by the hospital staff, to take care of patients with psychiatric and neurological problems, with a focus on neurocognitive disorders, all types of dementia and, special care for patients with Huntington’s disease.
I was assigned a key with a closet number, I was given a medical uniform, shoes, they made me feel at home.
Encuentro con el equipo clínico
Ya luego de dicha semana de adaptación, mi primer encuentro medico fue con el Dr. Jesús Ruiz Hidalgo, quien me presento a todo el equipo del Centro de Salud Polivalente Madre de Deu de la Merced, donde existe todo un equipo clínico: psiquiatría, neurología, neuropsicología, trabajo social, enfermería, auxiliares, así como personal de labores específicas, como cocina y limpieza, que hacen posible la atención humana y profesional que proporciona dicha institución para la atención a pacientes con problemas psiquiátricos y neurológicos, con efoque en los trastornos neurocognitivos, todo tipo de demencia y un especial cuidado a los pacientes con la enfermedad de Huntington.
Se me asigno una llave con numero de closet, se me suministro uniforme médico, zapatos, me hicieron sentir en casa.
Meeting with the patients
Dr. Jesús Ruiz Idiago was in charge of introducing me to admitted Huntington’s disease patients -and others- and explained to me the methodology of the work itself and the particularities of the system. Psychiatric medical care in Barcelona is not held in just one place, but it is spread over a wide network of connected outpatient clinics, day hospitals, acute units, long-stay units, home visits and scheduled visits. The entire network is at the service of guaranteeing the care of patients with mental disorders at all levels and, where possible, inserting them into society, treating and educating the family simultaneously.
El encuentro con los pacientes
El Dr. Jesús Ruiz Idiago se encargó de presentarme a los pacientes con enfermedad de Huntington ingresados y a los demás, y me explicó la metodología propia del trabajo y las particularidades del sistema. La atención medica psiquiátrica en Barcelona no se monopoliza en un solo lugar, sino que se extiende en una amplia red, conectada con ambulatorios, hospitales de día, Unidad de Agudos, unidad de larga estancia, visitas domiciliarias y visitas programadas. Toda la red se pone al servicio de garantizar la atención de los pacientes con trastornos mentales en todos los niveles y, en lo posible, insertarlos en la sociedad, tratando y educando a la familia simultáneamente.
Monday, Wednesday, Thursday
Monday, Wednesday and Thursday I begin to work with Dr. Jesús Ruiz Idiago and I meet Saul Martínez Horta, neuropsychologist of the movement disorder unit, of the neurology service of the Hospital de Sant Pau who, together with a team of neurologists and neuropsychologists, leads out the Huntington’s disease consultation on Tuesdays through a program called ENROLL, funded and managed by CHDI Foundation. The team also advises, educates, and cares for the medical needs of the patient.
At the Hospital de Sant Pau, residents of psychiatry, neurology, and interns from multiple areas of neurosciences, Aldo lead academic lives, which converts this program into a temple dedicated to knowledge and to the search of knowledge in the world of the brain.
Lunes, miércoles, jueves
Lunes, miércoles y jueves empiezo a trabajar con el Dr. Jesús Ruiz Indigo, y conozco a Saul Martínez Horta, Neuropsicólogo de la unidad de trastorno de movimientos, del servicio de neurología del Hospital de Sant Pau que, junto a un equipo de neurólogos y neuropsicólogos, lleva a cabo la consulta de la enfermedad de Huntington los martes a través de un programa llamado ENROLL. El equipo también asesora, educa y atiende las necesidades médicas del paciente.
En el Hospital de Sant Pau, hacen vida académica residentes de psiquiatría, neurología y pasantes de múltiples áreas de las neurociencias, que lo convierten en todo un templo dedicado al conocimiento y la búsqueda del saber en el mundo del cerebro.
A Dinner of dreams and projects
The beginning of this fellowship was decorated with a special welcome dinner with Ignacio Muñoz Sanjuán, President of Factor-H who, with warmth and sympathy, shared an exquisite dinner and toasted to me for an “excellent start”, at the Ohla Eixample restaurant. We also met with Dr. Jesús and Saul at the Tragaluz restaurant, where ideas, dreams and projects came up spontaneously as topics in the conversation.
It was an excellent evening to kick off a wonderful clinical learning and cultural exchange.

El inicio de este fellowship fue decorado con una especial cena de bienvenida con Ignacio Muñoz Sanjuán, presidente de Factor-H quien con calidez y simpatía, compartió una exquisita cena y brindó conmigo por un excelente inicio, en el restaurante de Ohla Eixample. También nos reunimos con El Dr. Jesús y Saul en el restaurante Tragaluz, donde ideas, sueños y proyectos salían en los temas de conversación de forma espontánea.
Una excelente noche para dar inicio a todo un maravilloso aprendizaje clínico e intercambio cultural.
